As a supervisor, you are responsible for keeping your workers or students safe while on campus. Supervisors are also responsible for conducting investigations and filling out all necessary forms in the event of an illness or injury involving someone they supervise.

CU WorkSafe offers the following advantages to supervisors:

• Receive notifications when hazards are identified in your work area.
• Track steps taken to correct issues or assign to the FMP service center (for maintenance issues).
• Follow-up with injuries, illnesses and incidents involving your staff/students and complete investigation reports online.

There have been no changes to the reporting process with the Workplace Safety Insurance Board (WSIB). The WSIB is still notified of injuries involving lost time and still receive Form 7s from Carleton University.

However, supervisors no longer need to fill out a separate Form 7 when submitting an injury report online. If the nature of the injury requires the WSIB to be notified, a Form 7 will be generated from the information included in the CU WorkSafe injury report and submitted to WSIB, as before.

Only Good Catch Reports can be submitted anonymously. To do so, please enter CU WorkSafe via the Guest Portal and select “Good Catch Report”.

Any Good Catch Reports submitted while logged in will not be anonymous.

We recommend using these 1-2 page help guides while completing your first few reports to ensure a clear understanding of what information should be included.

For our purposes, first aid is not considered healthcare. To be included in the healthcare classification, treatment must involve the care of a health care professional (e.g. doctor, nurse, paramedic, etc..).

Visiting a hospital, walk-in clinic, family doctor, etc.. are examples of instances where “Health Care” should be selected in the report form.

If you are unsure of which box to check, include a description of the care received in the notes section so the report can be amended if appropriate.

Photos and documents can be attached to Good Catch, Injury, Illness and Incident reports by either navigating to the “documents” tab or scrolling to the bottom of the page.

The file type you are attempting to upload may not be supported by CU WorkSafe. A list of supported file types is available on the Technical Information page.

As a student, you must access CU WorkSafe through the Guest Portal. Students cannot access CU WorkSafe with their MC1 credentials.

If you are a Carleton student, TA, visitor or contractor, you must access CU WorkSafe through the Guest Portal.
